What To Bring To Your First Appointment

      1. Complete the forms on the Patient Portal or download the forms on the Patient Forms page.

     

      1. Information from your referring physician
            • Including X-rays, MRI’s and CT scans.

           

            • The X-rays themselves will be returned as soon as possible after the examination.

           

          • Copies of any reports related to office visit. All hospital records, pathology and X-rays should also be brought with you.

     

      1. Insurance
            • Please have available at the time of your visit the following insurance identification:
              • Your insurance identification card so that we may copy the front and the back of the card for accurate insurance information.
              • If you have a health plan that requires their own insurance claim form, please bring us a signed completed claim form.

           

            • If you do not have insurance, payment in full is expected at the time of service. If you have a financial hardship, special financial arrangements may be made with our financial counselor to discuss a payment plan option.

           

          • We are participating Medicare providers. We accept Medicare assignment. If you have Medicare, we will bill your Medicare insurance and supplemental carrier for you. We will collect co-payments at the time of service and bill you for the incurred balance. In the instance that you are covered by Medicare and your secondary insurance is a PPO or HMO plan, we will handle the insurance billing for you with benefits assigned to our office.

     

    1. Prescriptions
      • A list of any and all medications you are currently taking including name of drug and dosage amount.

    Please be aware that sometimes our neurosurgeons have unexpected emergencies or surgeries that arise at the last minute and may disrupt our schedules despite our best efforts. We will try our best to contact you to reschedule your appointment. If you live outside Tulsa, we strongly recommend calling our office to confirm your appointment before you leave home.
